The Clinical Study on X-Ray Pelvimetry

Abstract
89 X-ray Pelvimetries by colcher-Sussman method within 2 weeks from E.D.C. or during labor since March 1968 till October 1968 were taken at the prenatal clinic and at the delivery ward of the Seoul National University Hospital, and had one ormore of the indications for X-ray pelvimetry.. of 89 cases, 51 delivered through the vaginal route, -and another 38 through cesarean section due to the C.P.D. (Contracted pelves and postmaturity)
I. The results of X-ray pelvimetry and cephalometry were as follows,
1) The measurements from 51 vaginal deliveries
A. Pelvic inlet M+a (cm)
i. Anatomical true conjugate: 11.60¡¾0.78
ii. Obstetrical true conjugate: 11.17¡¾0.52
iii. A-P diameter of actual inlet: 12.07¡¾0.60
iv. Transverse diameter: 12.78¡¾0.69
B. Mid-Pelvis
i. A-P diameter: 11.36¡¾0.87
ii. Interspinous diameter: 10.06¡¾0.54 -iii. Posterior sagittal diameter: 3.86¡¾0.58 C. Pelvic outlet
i. A-P diameter (post. sag. a.): 8.48¡¾0.69
ii. Intertuberous diameter: 10.35¡¾0.91 2) The measurements from 38 Cesarean Sections
A. Pelvic inlet
i. Anatomical true conjugate: 10.62¡¾0.95 ii. Obstetrical true conjugate: 10.17¡¾0.87 iii. A-P diameter of the actual inlet: 11.29¡¾0.78 iv. Transverse diameter: 12.32¡¾0.79 B. Mid-pelvis
L A-P diameter: 11.12¡¾0.64
ii. Interspinious diameter: 9.57-0. 6 6
iii. Post. sagittal diameter: 3. X0_0.51 C. Pelvic outlet
i. A-P diameter (post. sag. d.): 8.25¡¾0.74
ii. Intertuberous diameter: 9.92+0.84
The difference of measurements between cases of vaginal delivery and of Cesarean section was significant.
lI "=he measurements form the contracted pelivis
The incidence of Cesarean section was over 85% in 9.5cm or less of the obstetrical conjugate, and 71% in 9.5-10.0cm in the¢¥ inlet contraction. 5 of 8 cases who taken cesarean section due to the mid-pelvic contraction were 9.0cm or less of the terspinous diameter, another 3 were 9. 1 or more. Cesarean section was indicated 9.0cm or less (of the interspinous "diameter). - Outlet contraction was found rare.
Inlet and mid-pelvic contraction occupied about two thirds of the combined contraction.
I". Relationship between the height of body and obstetrical conjugate of the pelvic¢¥inlet Showed no valuable mutual relation.
IV: Relationship between pelvimetry and cephalometry the average skull diameter (A.S.D.). which was the mean value of the longest diameters and the shortest diameters, of the A-P . and lateral films was related to the fetal body weight. The cesarean section rate was depended upon,, the difference of the anatomical true conjugate and A.S.D. The incidence of . cesarean section was 53% in 1.0cm or less of the difference of the C.V.A.-A.S.D.
